The Coordinated Entry System (CES) is a system for matching people in need of housing that is appropriate for them. If you are interested in supportive housing, and have a disability, you should contact one of the community non-profits in your area listed below for an initial interview and assessment.
The Coordinated Entry System uses The Vulnerability Index – Service Prioritization Decision Assistance Tool (VI-SPDAT) to determine whether a client has high, moderate, or low acuity housing needs. This brief survey was developed as an assessment tool for frontline workers at agencies that work with unhoused individuals. This survey can be conducted alongside case managers while entering the CES.
To remain on Coordinated Entry housing list, you must remain engaged with your case manager to avoid being removed from the housing waitlist.
TMHA has two programs that can help you with CES referrals.
Behavioral Health Navigators (BHN) consists of a highly dedicated team of peers who strive to inspire others who are living with mental illness. They draw upon our unique experiences to provide guidance on the journey to recovery. Email BHNreferrals@t-mha.org or call (805) 540-6576.
PATH (Projects for Assistance in Transition from Homelessness) is a dedicated outreach worker contacting and engaging homeless individuals and family members not currently connected to mainstream services. Please call the Program Manager at 805 748-0717.

Work with a shelter, outreach worker, behavioral-health case manager, hospital social worker, probation officer, or other participating provider to complete the assessment.
Call 211, call 800-400-1572, or text the person’s ZIP code to 898-211. Santa Barbara County 211 can identify current shelters, eviction-prevention programs, rental assistance, food assistance, disability resources, and regional housing programs. It is free, confidential, multilingual, and available 24 hours a day.
Helpful tip:
For someone already enrolled in a behavioral-health program, the most effective request is: “I need a housing assessment, Coordinated Entry enrollment, and evaluation for supportive housing or an intensive program with housing navigation.”
